Superbill

If you are not using your insurance for services, I can provide you with a Superbill. A Superbill is a detailed receipt that includes the information your insurance company may need to process a claim. You can submit the Superbill directly to your insurance company to request possible reimbursement for your counseling services. Reimbursement is not guaranteed and depends on the benefits and requirements of your specific insurance plan. Please contact your insurance company directly to find out whether your plan provides out-of-network benefits for counseling services and what your reimbursement process may be.

Good Faith Estimate

You have the right to receive a Good Faith Estimate explaining how much your medical care will cost Under the law, health care providers, including mental health providers, need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. The Good Faith Estimate is an estimate of expected charges and is not a guarantee of the final cost of services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill with Sage Hope Counseling, LLC. You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS) within 120 calendar days of the date on the original bill. If the agency reviewing your dispute agrees with you, you will have to pay the costs listed on the Good Faith Estimate only.  If the agency reviewing the dispute disagrees with you, you will be required to pay the full amount billed to you.

  • Make sure to save a copy or picture of your Good Faith Estimate.
